PTA EXPENSE REIMBURSEMENT REQUEST PROCESS:

  1. Download the Expense Reimbursement Form here.
  2. Fill it out and attach your receipt. We recommend taping the receipt on a blank piece of paper and stapling them together.
  3. Return the completed form to the PTA Basket in the Library Office or email it to mindybrummettpullenptatreasurer@yahoo.com
  4. Our Treasurer will review the request and write a check to you.
  5. Our Treasurer will put your check in the PTA Basket in the Library Office or mail it to you (depending on what you check on the form).  She will email you when the check is ready and in the basket.
  6. Please allow 7-14 business days for PTA reimbursements.

PTA Disbursement PROCESS:

  1. Download the Disbursement Form here.
  2. Fill it out and attach the invoice that you need to have paid. 
  3. Return the completed form to the PTA Basket in the Library Office or email it to mindybrummettpullenptatreasurer@yahoo.com
  4. Our Treasurer will review the request and write a check to the vendor that you have specified.
  5. Our Treasurer will put your check in the PTA Basket in the Library Office or mail it to the vendor directly (depending on what you check on the form).  She will email you when the check is ready and in the basket, or has been sent.
  6. Please allow 7-14 business days for PTA disbursements.

PTA Deposit PROCESS:

  1. Download the Itemized Deposit Form here.
  2. Have two people count and record the amount of cash on the front of the Itemized Deposit Form.  Make sure that both counters sign the form!
  3. If you have checks, list them on the check receipt form and carry the check total to the top of the Deposit Form.
  4. Group all cash, checks and forms together to provide to the Treasurer.
  5. Contact our Treasurer at mindybrummettpullenptatreasurer@yahoo.com and arrange to meet her to give her your deposit directly.

 

PTA PLan of Work Form: 

  1. Download the Plan of Work Form here
  2. Fill out the Chairman Name, Position, and Year.
  3. Fill out the Responsibilities and Committee Members. 
  4. List the Specific Action Steps, Start Date, Completion Date (if available), and Budget

 

BACKGROUND CHECK FORM:
